Seroconversion

Seroconversion. After (2-4 wk) initial infection with HIV-1, a subset (40%-60%) of patients develop an acute mononucleosis-like illness (pharyngitis, fever, fatigue, headache, and lymphadenopathy) often associated with a faintly erythematous, nonpruritic skin rash that may be fleeting. This rash is generally more prominent centrally than peripherally and may involve the face. Oropharyngeal and esophageal ulcerations may also be noted. Thrombocytopenia, leukopenia, and abnormal liver enzyme levels are characteristic laboratory findings. One of the hallmarks of early HIV infection is elevated viral levels in blood and genital secretions. From the public health perspective, the diagnosis of early HIV infection is important, as these patients often continue to engage in high-risk behaviors while highly infectious. The rate of HIV transmission per coital act is highest during early infection.
